A: If yes, click here.
Q: Are you a Graduate Student needing to survey people to gather data for a Thesis or Dissertation?
A: If yes, click here.
Faculty, Staff and Group Accounts
Q: Are you a Faculty or Staff member working alone on this survey?
A: If yes, click here to request an Individual Account.
Q: Can a co-worker modify a survey in my individual account in my absence?
A: Providing your User ID and password to others breeches the security and confidentiality of all surveys, and the response data, in your account. It is best to consider requesting a Group Account.
Q: Will multiple people in your group or project, need the ability to design/modify/publish your project's survey(s)?
A: WebSurveyor allows for the sharing of surveys between account holders. This type of sharing has its limitations. In order to allow full access rights, to create, modify, publish and delete a survey, to more than one person, a Group Account is necessary. A group account provides the same right to each of its members. Please read the instructions that follow to begin.
Instructions for Group Account
- Create a Group Name relevant to the department or project, using 19 character or less (including the hyphens) and meeting the University established naming guidelines. If you would like us to assist you with this naming process, please let us know. This can be more easily accomplished, if you provide us with a brief desciption of the project and department that you are working with.
Group Account Name Examples:
CoM-Anesth-ResPrgm = College of Medicine, Anesthesia, Residency Program (college, department, program)
NRSG-ConEd-SelfStdy = College of Nursing, Continuing Education, Self Study (college, department, program)
OSL-Org-STAR = Office of Student Life, Organization, Student To Assist Recruitment (college, department, program)
- Name a person to be the Group Account Holder. The Group Account Holder will be the only person allowed to request a password reset and/or modify the group membership, in the event of employment changes or for other reasons. The account holder's name should match the name showin in the online Phonebook (for example, "Robert Smith" rather than "Bob Smith"). Consideration should be made as to the overall availability of the person in this role.
- Create a list of Group Members. This list must include each member name, as shown in the University directory (using either the online UI Phonebook/E-mail pages or the Global Address List (GAL) within Microsoft Outlook) and Hawk IDs for each group member.
- Once you have complete steps 1 - 3, you are ready to request a group account. To begin your request, click here. Please include all requested items (the information requested in steps 1-3 above) in your email correspondence.
Survey & Desktop Application Services will finalize the process by:
- Mail enabling the group
- Creating the WebSurveyor user ID and password
- Emailing a confirmation to the group e-mail address so that all members are notified of the WebSurveyor login information.
